Accident Reporting

The accident information package is found in each agency owned and motor pool rental vehicle. Each packet contains several index cards that list information pertaining to:
  • Gray card contains - state police emergency telephone numbers.
  • Green card contains - witness courtesy information.
  • Blue card contains - other involved driver information exchange.
  • White card contains - Identification Card for insurance purposes.
  • Pink card contains - Passing motorist assistance information.
REPORT ALL ACCIDENTS INVOLVING A VEHICLE *IMMEDIATELY CALL*
  1. Virginia State Police at 800-552-9965 or #77 on cell phone. The policy report is required when filing a claim.
  2. Crawford & Company Claim Service at 866-219-6120.
  3. The Driver's Agency Vehicle Coordinator
STOP IMMEDIATELY
Take all necessary precautions to prevent further accidents at the scene.
SEND FOR HELP - DO NOT LEAVE
Ask a passing motorist or some other person to contact the State Police or call 911.
GET NAMES & PHONE NUMBER OF WITNESSES
Fill out the accident report form and notify your supervisor.
DO NOT
Make a statement of any kind to anyone other than your employer, and enforcement officer, a representative of Crawford and Company or the Division of Risk Management.
